fix(web): uniform size for system grant cards - #111

Cards in the bot "System permissions" panel now share one width and height: the grid stretches every row to the tallest card, descriptions are clamped to three lines with the full text on hover, and the allow-write switch moves inside the card as a footer. Selected cards get a tinted background in addition to the primary border. Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
