Update md_eval_22 with the following:

Make the check that the average across the individual examples matches the overall reported error rate ("ALL") more permissive. Fixes Safety check on average DER fails #121
Call md-eval-22 with the -a f option to obtain per-session results with a single call to md-eval-22.pl
