This PR updates the project’s testing/CI environment and README guidance to reinforce and document the project’s 100% code coverage expectations.
Changes:
- Add a README code coverage badge linked to the Tests section.
- Normalize README shell command snippets (remove
$prompt) and document how to generate local coverage output with Xdebug. - Update GitHub Actions CI workflow to use
actions/checkout@v7.
