docs(T-081): record landed vs remaining after #145#160

The raster describe path is structured; the diagram path still calls the client directly, and neither cache fingerprint covers the response format (an open acceptance criterion — the structured prompt drops the [SKIP] instruction, so pre-#145 caches are silently reused). Handoff hygiene after the #145 merge.
partially landed (#145).call_vision, and neitherimage_fpnordiagram_fpcovers the response format — an open AC, since the structured prompt drops the[SKIP]instruction and pre-feat(describe): structured output for the skip decision (T-081, keystone) #145 caches are reused unchanged.Docs only.
