test(runtime): pass iteration budgets to raw effect-machine calls
2ad9250 
The counted-for frames slice made the loop iteration budget mandatory
for raw machine entry (fail-closed, mirroring the envelope's
accepted.limits.maxCollectionLength wiring in execute.ts), but the
pre-existing direct-call while/for tests never supplied one, so every
loop test threw 'effect machine loop requires an iteration budget'.
Supply the shared fixture limit at each raw call site; envelope-path
tests already exercise the budget and stay unchanged.

test(runtime): budget the false-condition while fixture too

Review follow-up: the false-initial-condition test passed only because
the budget is consumed after the condition check; give it an explicit
budget so no raw loop entry is unbudgeted.

fix(runtime): address guard findings on capability seam
5401c5e 
State the intent of the deliberately swallowed stray-promise rejection
in sync interceptor mode, and make the capability result-binding check
narrow via an explicit typeof before the portable-name guard so the
narrowing survives analyzers that cannot resolve imported type guards.
No behavior change; real tsc was already clean.
