Introduce git commit message template and PR template #192

Description
This repository does not contain a git commit message template or a PR template. It is good to have them in a repository. Plus, the workflow will fail if we do not have a Signed-off-by statement in the git commit message.
Note: We can have commitlint, if we want to enforce a commit message standard. But for the scope of this issue, let's stick to introducing just a commit message template.
Acceptance criteria
- Add a git commit message template
- Add a Pull Request template
