Refactor and fix rules for 'ref', 'cite', 'vref' and 'vcite'

1. There is no rule for vcite, despite it being mentioned in a comment
2. Rules for ref, cite and vref are all generated differently, while all of them can be generated using SimpleCommandState class.
3. Rule for cite is wirtten twice with different token types.
4. Token types for ref and vref are different.
