Workers can disconnect from experiment without callback being triggered

Strange "stealth disconnect" creates complications when tracking experiment.

Possibilities

• Cometd is not doing a callback for all removed sessions (should report)
• Callback is not making it through TurkServer to the experiment. For now, added worker reconnect and disconnect functions before experiment check. Possible that map/unmap is broken somehow - worker map being removed?
